Description:
The Physical Therapist has a direct impact on delivering care, by evaluating and treating patients with disabilities, injuries and or diseases. In this position, you will work with our patients to achieve established patient goals, Design treatment plans and will participate in chart reviews and activities to help ensure consistent quality in our patient care.

Responsibilities include:

• Evaluating new patients through questioning and observation and developing treatment plans based on assessments.
• Utilizing physical and mechanical means in accordance with the written and signed prescription and/or referral of a physician or authorized practitioner, in order to and treat patients.
• Making accurate observations of patient tolerance and/or status during treatment and demonstrating the ability to modify a treatment program as needed.
• Maintaining complete and up to date medical records for each patient.
• Submitting required documentation in accordance with the policies and procedures of the department.
• Working with other physical therapists to; maintain the highest standards of care, while improving patient compliance and retention.

Qualifications:
Bachelor's Degree in Physical Therapy required. Master's/Doctorate Degree, preferred.
