Job Description:

The Parenting, Independent Living Skills (ILS) Instructor position provides personal instruction, care and direct care support to developmentally adults or seniors in their home and community.

This may include instruction and assistance of personal care, light housekeeping, recreational activities, transportation, meal preparation, community access and other duties as outlined in the Individual Service Plan (ISP).

Instructor will implement the consumers vision and choices as stated in the ISP. The Instructor shall be aware that the consumers, as well as LeBlanc, are At-will employers, meaning that employment may be terminated at any time by the employer or employee with or without cause.

Job Location:

We provide services all over North Bay and East Bay, San Francisco , Hayward, Union City, Fremont, and surrounding cities.

Job Duties:

  1. Instruct and Assist the consumer(s) in: personal care, such as feeding, meal preparation.

  2. Support and guide the consumer in the choices they make.

  3. Assist or facilitate: medical appointments, communication, etc.

  4. Assist with planning and/or finding activities that lead to community involvement .

  5. Provide companionship, guidance and supervision during such activities.

  6. Assist clients with basic housekeeping tasks, laundry and grocery shopping.

  7. Perform documentation according to policies and procedures.

  8. Ensure consumer health and safety during assigned hours of responsibility.

  9. Assist in the overall maintenance of the consumer's home and general upkeep.

  10. Perform all duties as specified in the ISP and the consumer orientation checklist.

  11. Attend Interdisciplinary Team Meetings,.

  12. Cooperate with other in home services.

  13. Report suspected abuse or any unusual incident as indicated in the Training Manual.

  14. Complete and submit time log, mileage in a timely manner.

  15. Perform all duties in a safe manner.

  16. Report workplace safety issues, consumer injuries, consumer threats to supervisor.

  17. Complete LEBLANC training curriculum and other training.

  18. Other duties as assigned by supervisors.

  19. Abide by all agency policies and procedures as indicated in the Employee Handbook.

Requirements:

High School Diploma or equivalent;
Mandarin/ Cantonese or Spanish Speaking;
Reliable transportation and valid Driver’s License;
Negative TB test result;
Pass criminal background check;
Optional Requirements: (Preferred Not Required)

Experience working with people with developmental disabilities;
Computer literate and experience with MS Office suite of products;
Valid CPR/First Aid License. (We will train you);
Skills Preferred : (Not Required)

Knowledge of personal health-care practices and principles
Knowledge of the principles of home management;
Knowledge of the elements of nutrition and meal planning;
Knowledge of first aid skills including CPR and home safety;
Knowledge of the aging process, developmental disabilities.
Knowledge of the emotional problems accompanying illness;
Knowledge of vulnerable adult reporting laws.
Skill in using appropriate lifting and transfer methods with clients;
Ability to accept and adapt to varying life styles and home environments.
Ability to follow oral and written directions.
Ability to retain confidentiality of home conditions and situations;
Ability to communicate effectively orally and in writing;
Ability to establish and maintain effective working relationships with clients;
Ability to work under stressful conditions and to remain calm and objective.
Basic computer skills: Microsoft word, excel, and internet.
Ability to type case notes, fill out required forms, schedule appointments a, type correspondence and manage a load of 5 or more clients.
Self Discipline:

This person must be a professional with the ability to work independently posing time management efficiency, integrity, ambition, loyalty and motivation. Communicate well, with clients and management.
