Responsibilities
Develop and test (through on-the-ground pilot programs) protocols and processes for therapists assisting primary care providers, community health workers, pharmacists and care coordinators to improve care outcomes for patients receiving Medicaid
Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate
Work closely with a mobile community healthcare worker team
Hire, mentor and supervise junior therapists to act as behavioral health care providers and coordinators
Communicate with patients and providers through multi-modal CRMs including multiple EHRs, text, phone, and video
Participate in QI activities to assure quality, cost-effectiveness, and ROI
Minimum qualifications
Licensed Mental Health Counselor/Professional Counselor or Licensed Marriage and Family Therapist or Licensed Social Worker with good standing in Washington State
10+ years of experience in behavioral healthcare, including delivery of Collaborative Care Model among vulnerable populations
Experience in multi-disciplinary care management
Positive references from prior mentees and supervisors
Preferred qualifications
Collaborative Care Model certification and experience
Experience providing evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated
LEAN Six Sigma/QI experience, with Green Belt or above Lean certification
Strong analytic skills and ability to work with multiple data registries for proactive patient outreach
