Who would be an ideal candidate?
We are looking for a talented and passionate Behavioral Health Care Manager with experience who will offer monitoring and clinical support for Rose users. Our goal is to keep the clients in the least restrictive and most cost-effective mental health care appropriate to their needs, but to also deliver top-notch clinical support when indicated.
Key Responsibilities:
● Review new and returning users' mental health data in the Rose platform, flag users requiring follow-up, and summarize notes to a provider
● Conduct on-going remote patient monitoring through the Rose platform. Monitor treatment compliance, changes in symptoms, and any potential treatment side effects
● Communicate with a variety of doctors/therapists as needed through the Rose platform
● Maintain and enhance behavioral health care manager competence through regular delivery of services and professional development opportunities
● Liaise between patient and provider to comprehensively coordinate mental health care
● Provide in-app wellness checks and crisis interventions for users, assign targeted psychoeducational content
● Track assigned users in a registry spreadsheet, ensuring timely follow-up so no one falls through the cracks
● Aggregate results for outcome research and general feedback
About You:
● You have a Masters’ degree in Psychology, Social Work, Counseling, or Nursing
● You have 2+ years of post-graduate experience in psychotherapy and/or case management
● You have a clinical licensure, especially LCSW or RN with specialization in behavioral health
● You feel comfortable working in a high-paced environment juggling multiple patients
● You are organized, punctual, and have excellent written and verbal communication skills
● You are able to work well independently and be easily accessible by phone and email
● You are passionate about using technology and innovation to improve access to behavioral health services
You will report to the Clinical Director and participates in weekly caseload review and team meetings, as well as continuing education seminars
